For Corporation or Limited Liability: List all Officers, Directors, Members and Majority Stock Holders

Please do not list subcontractors, revolving credit accounts, or utilities. (Minimum of 3 Required)

Electronic Invoicing is a standard procedure here at BII. All customers will be electronically invoiced within days of the shipment. Your original invoice will be emailed to the addresses you provide below. Typically these addresses are one A/P and one Buyer.
